Registration Form
Steps for Registering your student
1. Click on REGISTRATION FORM (above) and fill in the information. When you are done, click on the Submit button.2. Click on BACK OF REGISTRATION FORM and print it out. Read and initial and sign the form and send it into Kucinski Academy, Morningside College, 1501 Morningside Ave, Sioux City, IA 51106, along with the first months payment.
3. If you are in need of a scholarship, click on NEED BASED SCHOLARSHIP FORM and print it out. And then send it into the Kucinski Academy along with the first 2 pages of your tax return from last year.
The first payment is due when registering. There is no refund for missed lessons. Lessons will take place at the MacCollin Classroom Building on the Morningside College campus.

Tuition covers 15 lessons per session, recitals, performance classes and other Academy events. Fees are payable in two installments each semester. Enrollment is for the school year. Students starting mid-year will be pro-rated. Fees can be paid in the Morningside College music office between 8-5, left in the locked drop box next to the Academy bulletin board, or by mailing the check to Leo Kucinski Academy of Music, 1501 Morningside Ave., Sioux City, IA 51106.Scholarships
Need-based scholarships are made possible this year by a grant from The Gilchrist Foundation. The scholarships are restricted to students who are 21 years of age and younger.Scholarship Application and Qualifications
We have revised the scholarship structure for Leo Kucinski Academy of Music. Qualified students are eligible to be awarded one scholarship per semester. The $10 academy registration fee must be included with the scholarship application.Scholarships will be awarded on a semester-by-semester basis with each scholarship reviewed for renewal at the end of each semester. Scholarships are applicable to fall and spring semesters only.
If a student misses 3 lessons (unexcused) their scholarship may be revoked at the discretion of the teacher and the executive director.
Each scholarship application must be accompanied by a copy of the 2008 federal tax return of the student’s custodial parent(s) or guardian(s). Maximum scholarships may be awarded to those students with a household adjusted gross income (AGI) of $25,000 or less per year. Partial scholarships may be awarded to students with a household AGI of no more than $40,000. Applications submitted without the appropriate tax forms will not be considered.
Students on maximum scholarship will pay $5.00 for each 30-minute lesson and $7.50 for each 45-minute lesson. See registration form for payment of fees. Scholarships are not given for 60-minute lessons.

Example: AGI of $17,000 – a student would pay $120 for 24-30 minute lessons ($5.00 per lesson, scholarship of $330 to cover the remainder of the total cost)
Instrument Loan Program (available for need-based scholarship recipients)
• Instruments may be rented on a per-semester basis
• Semester rental is $2.00 per week for a 12-week semester for a total of $24 per semester
• In addition to the rental fee, a $76.00 deposit will be assessed at the time of rental
• The deposit will be refunded at the end of the semester provided the instrument is returned in the same condition
• Any instrument repair fees will be subtracted from the rental deposit • Fees are due at the time of rental
