Application for Admission to Graduate Study

A graduate student may apply for admission to graduate study before taking any classes or during/after completing up to nine graduate hours.  Please note that an original signature is needed on this application form.

Application for Admission to Candidacy

A graduate student may apply for candidacy after taking at least 18 graduate hours, including EDUC 501 Educational Research. This application needs to be submitted to the Graduate Office before October 1 of the school year you plan to graduate.  Please note that an original signature is needed on this application form.

SPECIAL EDUCATION INTERNSHIP APPLICATION FORMS 

PLEASE NOTE

SPED internship applications for Fall 2010 are due April 15, 2010.

Reminder:  Before an internship application is considered, the applicant must be admitted to the Graduate Program.  An original signature is needed on each of the internship application forms.

Note:  If you are completing the IS1 or IS2: MD program through the Iowa Strategist Network, there are different internship applications for these programs.  Please contact the Graduate Office for the appropriate alternate application.
